There are 2 forms. The first form is for everyone that has been involved in an auto accident or other personal injury. The second is the Basic Patient Information Form. It is for everyone else including those with insurance.

Please print out the necessary forms, fill them out, and bring them with you to your first visit. From there, Dr. Stone can talk to you and develop a specific plan of what may work best for you.

If you have never been to a chiropractor before or are unsure if this is the approach you’d like to take, we always offer an initial consultation for free. The initial consultation lets you explain what is going on and see the available treatments. We look forward to working with you.
